What You’ll Do

• Lead and chair Multi-Disciplinary Team (MDT) meetings.
• Conduct CHC reviews and assessments virtually via MS Teams or Zoom.
• Communicate complex clinical information with empathy and professionalism.
• Produce clear, person-centred reports for both clinical and non-clinical audiences.
• Work with a Integrated Care Board (ICB) to improve CHC delivery.

What You’ll Need

• NMC Registered Nurse with at least 12 months post-qualification experience.
• Strong written and verbal communication skills.
• Knowledge of CHC Framework (2022), Care Act (2014), and Mental Capacity Act (2005).
• Proficiency in Microsoft Outlook, Excel, and remote working tools.
